Eficiencia asistencial y bienestar del personal sanitario: Revisión sistemática sobre lean management en servicios de emergencia hospitalarios

Resumen
La implementación de Lean Management en los servicios de emergencia hospitalarios se ha extendido como una estrategia para optimizar procesos, reducir desperdicios y mejorar la eficiencia asistencial. Sin embargo, sus efectos sobre las condiciones laborales y el bienestar del personal sanitario siguen siendo objeto de debate, especialmente en entornos caracterizados por alta presión, sobrecarga, decisiones urgentes y exposición constante a situaciones críticas. El objetivo de este artículo fue analizar la evidencia científica disponible sobre los efectos de la implementación de Lean Management en las condiciones laborales y el bienestar físico y psicológico del personal de los servicios de emergencia hospitalarios. Se desarrolló un artículo de revisión sistemática, siguiendo lineamientos metodológicos orientados a la identificación, selección y análisis crítico de estudios científicos relacionados con Lean Management, Lean Healthcare, Lean Thinking, mejora continua y servicios de emergencia. La revisión permitió organizar la evidencia en torno a cuatro ejes: bienestar psicológico, condiciones laborales, factores mediadores o moderadores y estrategias de mitigación. Los resultados muestran que Lean puede generar efectos positivos cuando reduce tiempos de espera, mejora el flujo de pacientes, ordena tareas, fortalece la comunicación y favorece la participación del personal. No obstante, también se identificaron efectos adversos cuando la metodología se implementa de manera instrumental, centrada solo en indicadores de eficiencia, sin considerar la carga laboral, la autonomía, la privacidad, la fatiga y el desgaste emocional del equipo. Se concluye que Lean Management no debe entenderse únicamente como una herramienta operativa, sino como una intervención organizacional que requiere liderazgo, soporte institucional y protección explícita del bienestar del personal sanitario.
